2010/2011 Risk Management Procedures
New Coaches and those who have not completed the online registration
Our by-laws and state laws require everyone who coaches children to complete a background check through an approved agency. We are now using Mid State Soccer Associations' on-line form to comply with this requirement. Board-Approved Coaches will have to use their credit card to pay the $30 fee. The Club will reimburse you for this expense.

Fill out disclosure form and submit.
You will need to use your credit card. The club will reimburse you.
Print out 3 copies of the disclosure form.
Submit 2 with team registration and keep one for your records.
